Tender Kisses Nursing Care is a home visitation program that provides in-home nursing care and case management to new moms and babies to prevent infant, child, and maternal mortality, abuse, and neglect.
Specially trained pediatric and maternal nurses make visits to the home environment of pregnant women or parents with babies who may face medical, social, or environmental risks.
During the Covid-19 Pandemic, visits can also be conducted via audio and video.
At the initial home visits:
During Pregnancy:
TKC nurses will complete a health assessment of the expectant mom - as well as screening for risk factors including maternal depression and domestic violence.
After Delivery:
Utilizing evidence-based clinical guidelines, TKC nurses will complete a postpartum assessment on mom and assessment of the baby to prepare a culturally appropriate plan of care for mom, baby, and family during the "fourth trimester - ensuring that health, safety, development, and wellness needs are met for both infants and mother.
In addition, TKC nurses will help families access external community resources needed to identify and address any issues, find support the family may need, and provide health education to improve parenting skills and overall health, safety, and well-being.
Subsequent home visits will include:
Monitoring and evaluating mom and baby’s health and developmental progress with the plan of care nurses created with the family at the initial home visit.
Services are provided as needed to moms and new babies and may be provided for up to two years after the baby’s birth. TKC nurses will also collaborate with other community agencies to provide the most appropriate care to families.
